AN APPOINTMENT?
Appointments can be made by telephoning 01349 863033 or 863034 or by calling in person at the health centre. Routine appointments can usually be made one month in advance, which enables us to offer you an appointment at a time to suit you. If you require an urgent appointment, please telephone as soon as possible after 8.00am. Remember: if you are unable to keep your appointment, please let us know immediately so we can offer it to another patient and avoid delays for everyone. We operate 48 hour access where patients are guaranteed contact with a health professional within 48 hours. Please mention this to the reception staff if you wish to use this facility.

prompt. If the situation is critical, patients should feel free to dial 999 and

OBTAIN
A HOME VISIT?
Home visits are for patients who are housebound through illness or disability. If you require a home visit, please telephone 01349 863033 or 863034 before 10.00am and be prepared to give the receptionist as much information as possible. This allows the doctors to plan their rounds most efficiently and to allocate priority. If you can get to the health centre, please do so as several patients can be seen in the time it takes to do one home visit.

OBTAIN
REPEAT PRESCRIPTIONS?
Repeat prescriptions will be issued at the doctor's discretion and are normally for patients on long-term treatment. You will be issued with a computer slip attached to your prescription. When you require a further supply, the following methods of requesting routine repeats are available, and patients are requested to use one of these:
1. Deliver your repeat prescription re-order slip to the Health Centre
2. Post your repeat prescription re-order slip to the Health Centre
3. Deliver your repeat prescription re-order slip to Boots or Lloyds Chemists, Dingwall
4. Fax your request to the Health Centre on 01349 862022
5. e-mail your request to High-UHB.GP55376-Admin@nhs.net with ‘repeat prescription request’ as the subject heading.
Please assist us by using the repeat script form rather than telephoning your request. This will reduce the number of errors and allow us to deliver a more efficient service, keeping the phone lines free for emergency calls. Please allow two working days for us to process your request. Prescriptions can be collected at the health centre, posted out to you (if you enclose an SAE ), or delivered directly to local chemists if you tell us which chemist you use.

REGISTER
AS A PATIENT?
Patients wishing to register should
bring their NHS medical card and medical cards for anyone else in their family
whom they also wish to register. If you don’t have a card the receptionist
will ask you to complete a form for each person registering with the practice.
In addition you will be asked to complete a questionnaire about your medical history
to help us until your medical record arrives from your previous doctor.
If you change your name, address
or telephone number please notify the surgery as soon as possible.
If
you move out of the practice area you will have to register with a new practice.
