We are writing to all our patients whom we regularly review as part of our Chronic Disease Management process. A chronic disease is a disease or other health condition that is persistent or long lasting in nature, and in particular we regularly review our Asthma, Diabetes, Chronic Chest Disease, Chronic Heart Disease and Stroke/TIA patients.
Since February 2011, when we migrated to a new computer system, we have been reviewing the way in which we contact and call patients in for Chronic Disease Management review. From January 2012 new procedures will be fully in place. The following outlines the changes in the way we will review chronic disease in future
It is really important that chronic disease patients are reviewed at least once a year. From now on we would like the patient themselves to make an appointment for their review without prompting from us. To make this easier to remember we are adjusting review dates to be inthe same month as the patient's birthday. (Diabetic patients will also have another review 6 months later)
In the month of their birthday, the patient should telephone the surgery to make an appointment. Staff will ask which disease(s) they suffer from, refer to their notes if necessary, and make an appointment with the appropriate clinician i.e. doctor, nurse or both. All our chronic disease management clinics are nurse led.
If physically unable to attend the surgery, we may arrange a home visit, but patient's efforts to attend the surgery will be appreciated
A reminder may be sent from the Practice if they forget to attend, however patients should not wait for nor rely on receiving a letter.
As it is important that patients attend for review, and as a last resort, we may also limit their prescriptions until they attend.
Patients should feel free to contact the surgery if they have any queries regarding the new procedures.
Wart Clinics

Following the wart clinic on the 3rd June 2011, there will be no further nurse run wart clinics held at Dingwall Health Centre ·for advice re warts and verrucas please see one of the local pharmacists in the first instance. ·for multiple warts or verrucas, those on the face, where the diagnosis is in doubt or those resistant to treatment, after six months of daily over the counter treatment, patient may wish to make an appointment to see a doctor. Further useful advice re warts can be found at www.patient.co.uk/health/warts-and-veruccas.htm, and the reception desk at the Health has a small supply of printed material from the above web site for those patients who do not have internet access
Missed Appointments Policy

Whilst we appreciate that are occasions where an appointment can be simply forgotten or other circumstances that are out with your control may prevent you from attending, there are an increasing number of patients who make an appointment with a GP or practice nurse and then fail to attend or provide sufficient notice of cancellation. Every month, Dingwall Health Centre has around 180 missed appointments which equates to about a week and a half of work for a doctor or nurse. This obviously has a major impact on our ability to provide appointments to other patients.
Cancelling an appointment is easy to do, please call the receptionists on 01349 863034 or email us on High-UHB.GP55376-Admin@nhs.net. It is most useful if the appointment is cancelled as soon as you know you are not able to attend, therefore giving as much time as possible for another patient to take the opportunity.
As this problem is worsening, making it difficult for people to get appointments when needed, the practice will now be actively monitoring missed appointments. Patients who miss 2 appointments or fail to give sufficient notice that they will not be attending will be sent a warning letter as a reminder of their recently missed appointments. If appointments continue to be missed after this letter has been received, the matter will be brought up at a Practice meeting and action may be taken to limit the times the patient is able to book an appointment or possibly even removal from the practice list depending on the severity of the abuse of the appointment system.
